When: 6 Mondays, 9:00am – 12:00pm, Starting April 29
Where: Sabathani Community Center, Room 118 

Join us for a transformative 6-week journey toward better health management. Gain valuable insights and practical skills to enhance your well-being and overcome health challenges. This course combines expert presentations, interactive discussions, and engaging activities tailored to your needs. 

What You’ll Learn: 

  • Effective pain management strategies
  • Optimizing medication usage
  • Enhancing strength, flexibility, and endurance through exercise
  • Nutrition guidance for better health
  • Communication skills for interacting with family, friends, and healthcare providers
  • Evaluating new treatment options 

What You’ll Get: 

  • Weekly action plans for continuous progress
  • Peer support and problem-solving sessions
  • Opportunity to receive a gift card upon completing all sessions

This program is designed for individuals at high risk of developing chronic illnesses like diabetes. You are eligible if you have any of the following:

  • Prediabetes
  • Overweight status
  • Hypertension
  • Family history of type 2 diabetes
  • Low physical activity levels
  • Belonging to specific ethnicities: African American, Hispanic or Latino, American Indian, Alaska Native, Pacific Islander, or Asian American
