- Enhancing strength, flexibility, and endurance through exercise

- Communication skills for interacting with family, friends, and healthcare providers

Eligibility:
This program is designed for individuals at high risk of developing chronic illnesses like diabetes. You are eligible if you have any of the following:
- Prediabetes
- Overweight status
- Hypertension
- Family history of type 2 diabetes
- Low physical activity levels
- Belonging to specific ethnicities: African American, Hispanic or Latino, American Indian, Alaska Native, Pacific Islander, or Asian American
