Healthy Aging & Wellness Program 

Supporting healthy aging, independence, and well-being for adults 55 and older. 

Sabathani Community Center’s Live Well at Home program helps older adults connect with health and wellness services, educational opportunities, and community resources that support their well-being and independence. 

Through personalized assistance, health education, and connections with community partners, the program helps participants access the information and support they need to live healthier, more connected lives. 

Services Offered 

  • Health & Wellness Support: Guidance and resources to support overall health and well-being. 
  • Community Health Worker Assistance: Individualized help navigating health and community services. 
  • Health Education & Workshops: Educational sessions focused on healthy aging, wellness, and preventive care. 
  • Health Screenings: Opportunities to participate in screenings and preventive health activities. 
  • Community Resource Connections: Assistance finding healthcare, social services, and other resources for older adults. 
  • Information & Referrals: Connections to healthcare providers, community organizations, and senior-serving programs. 

Who Can Participate? 

Live Well at Home serves adults age 55 and older. 

Services are provided based on individual needs and program availability. Some activities or partner services may have additional eligibility requirements. 

How to Access Services 

Live Well at Home services and activities are offered throughout the year at Sabathani Community Center and other community locations.
