Supporting Youth Wellness in the Heart of South Minneapolis

Communities That Care (CTC) is a community-led prevention framework focused on supporting youth wellness and reducing risk factors that can lead to challenges such as substance misuse and other problem behaviors.  

At Sabathani, CTC brings together community leaders, parents and caregivers, youth, professionals, and other partners to use local data, lived experience, and proven prevention strategies to strengthen positive outcomes for young people in South Minneapolis. 

What is Communities That Care? 

Communities That Care is a tested, evidence-based framework that helps communities: 

• Use data to identify local strengths, needs, risk factors, and protective factors 
• Choose effective programs, practices, and policies that support youth wellbeing 
• Build shared community leadership around prevention and positive youth development 
• Create long-term, community-informed strategies for healthier futures

How Communities That Care Works at Sabathani 

Sabathani serves as a local home for the Communities That Care process, helping bring people together to listen, learn, plan, and take action. 

Key Leader Board 
The Key Leader Board includes community leaders who help champion the CTC process. This group helps identify who needs to be involved so the broader Community Board reflects the experiences, perspectives, and priorities of South Minneapolis. 

Community Board 
The Community Board includes professionals across sectors, parents and caregivers, youth, and community members interested in youth wellness and wellbeing. The Community Board meets regularly to review data, identify priorities, form work groups, and recommend programs, policies, and strategies that support positive youth outcomes.
