Sabathani’s Senior Outreach Services program supports African American and African seniors age 55+ in South Minneapolis by connecting them with personalized support, community resources, and meaningful social connections that help them remain engaged, informed, and independent.

Senior Outreach Services is available to:

- Residents of South Minneapolis
- Seniors seeking support, connection, or assistance accessing resources
How We Can Help
One-on-One Support
Meet with a Senior Outreach Coordinator to discuss your goals, needs, and priorities. Together, we’ll create a personalized service plan tailored to your situation.
Friendly Companionship
Connect with a trained volunteer companion for regular visits, conversation, activities, and social engagement.
Caregiver Support
Family caregivers are invited to participate in monthly Caregiver Circles, offering encouragement, shared experiences, and practical support.
Benefits Navigation
Get help understanding and applying for programs and benefits, including:
- Medical Assistance
