Helps prevent homelessness by offering: 

• Direct referrals from individuals via email or phone. 

• Referrals from Hennepin County via the RentHelp system. 

Eligibility: 

• Must live in Hennepin County, preferably near the 38th Street corridor. 

• Household income must be below 200% of the Federal Poverty Guidelines (FPG). 

• Support is limited monthly. If we’re unable to assist, referrals are made to other providers. 

Income Guidelines (200% FPG): 

Supports youth aged 16–24 who are referred through Coordinated Entry. 

Services Include: 

• Assistance obtaining and maintaining housing. 
• Ongoing case management.
• Maintaining and increasing financial stability.
• Improving social connection. 

For chronically homeless adults who: 

• Have been homeless for 12+ months OR homeless 4 times in 3 years (totaling 12+ months). 
• Are referred through Coordinated Entry.

Services Include: 

• Subsidized rental assistance. 
• Help obtaining and maintaining housing. 
• Ongoing case management. 

Helping families with school-age children stay housed and stable. 
The School to Housing (STH) program supports families who are at risk of homelessness or currently experiencing housing instability. We currently serve families in Bloomington and Richfield school districts.

We can only take clients into our STH program through referral from schools. 

You may qualify if you: 

• Are at or below 30% of the Area Median Income (AMI) 
• Are doubled up or living in your own unit 
• Have past-due rent or utility bills 
• Have school-age children enrolled in one of the eligible school districts 

Services Include: 

• Rental and utility assistance 
• Housing placement support 
• Ongoing case management

   Ensure you have school-age children enrolled in one of the eligible districts and meet the income and housing criteria listed above. 
2. Visit the RentHelp Webpage 
   Go to: renthelphennepin.hdsallita.com/home 
3. Apply for Assessment 
   Select the “Apply for Assessment” option to begin the process. 

Note: Sabathani accepts participants into the STH program through referrals from school social workers.
